POSITION TITLE: Program SupervisorCommunity Participation Supports (CPS),In-Home and Community Supports (IHCS),& Supported Employment (SE)Education Requirement:Associate's degree in relevant studies (Social Work, etc.); commitment to completing bachelor's program in relevant studies; 1 year of relevant ID/A experience, and commitment to complete ACRE training and CPS training within 90 days. STATUS: ExemptHOURS: 40 hours per weekSCHEDULE: 8:00 am to 4:30 pm, one-half hour lunch dailyThe hours indicated above are the general hours that the selected employee may expect to work. This in no way guarantees that these will be the sole hours or days that a staff will always work. Demands of the individuals or needs of the Agency that result in alterations of the schedule that employees work will be met first. As such, it is expected that this position will cover on-call needs as they arise. While it is the intention of the agency to limit additional work hours, on-call coverage needs will be a priority for this position. SCOPE OF DUTIES:This position is of supervisory/administrative nature. The position requires independentjudgment, organization and management skills. Responsibilities are performed underthe direction of the VP of ID Services.The CPS Program Supervisor is responsible for the leadership, coordination, and oversight of a Community Participation Supports program serving individuals with intellectual and developmental disabilities under the Pennsylvania Office of Developmental Programs (ODP). This role ensures the delivery of high-quality, person-centered, community-based services that promote independence, inclusion, and meaningful participation in community life. The supervisor ensures compliance with ODP regulations, organizational standards, and best practices while supporting staff and fostering positive outcomes for individuals served.ESSENTIAL JOB FUNCTIONS:Review agency policy and procedures, including program-specific practices, with new employees; review DPW regulations with new employees; and assure that as-needed training is conducted for existing employees. Refresh employees on policies and procedures when the need is identified.Ensure program-specific compliance with relevant regulations; report regularly to the Program VP of ID Services.Review ISPs and develop treatment plans. Participate and provide input in program-specific meetings for individuals and ISP. In conjunction with CPS, IHCS & SE staff, ensure that everyone receives all programmatic services available to meet his/her needs.Conduct quarterly review of the individuals' ISP (Individual Support Plan), all bi-annual individuals' assessments, and all Behavior Plans for individuals.Monitor utilization of service; follow up for any modifications to plans or delivery.Assist in completing assessments and evaluations of existing and prospective individuals.Work in conjunction with Staff Development Specialist in providing general orientation for new employees. Review daily operation of program services and requirements with new employees.Escort new employees to existing sites to meet staff and individuals.Regularly observe the delivery of services to consumers. This position requires regular and on-going physical arrival at consumer locations for supervision of staff and other assessment and monitoring.Assess programmatic needs of CPS, IHCS & SE programs and report findings to VP of ID Services semi-monthly.Assure that services meet billing criteria.Review and approve service notes.Participate in on-call duties as they arise. This position is required to fill in for Direct Support Professionals, sometimes across programs, as necessary regarding staff shortages.Evaluate daily routines and procedures as provided to each consumer and report on the programmatic implications to the VP of ID Services.Complete programmatic paperwork for all individuals assigned and report on completion to the VP of ID Services.Review programmatic records and reports received from each employee and maintain appropriate files and records of each. Provide problem solving assistance as needed.Prepare HCSIS semi-annual information to all counties as required.Develop and revise policy and procedures for CPS, IHCS & SE programsAssure that services meet billing criteria.Review and approve service notes.Participate in on-call duties as they arise. This position is required to fill in for Direct Support Professionals, sometimes across programs, as necessary regarding staff shortages. Use appropriate agency procedures for reporting concerns and/or complaints.Assist in maintaining effective professional relationships with internal and external sources.Ensure and safeguard confidential treatment of consumer protected health information in written, oral and electronic form maintained and obtained during your work with Lakeshore Community Services, Inc. PROBLEM SOLVING Demonstrate sound judgment by taking appropriate actions regarding questionable findings or concerns. Use appropriate agency procedures for reporting concerns and/or complaints.Consistently evaluate work and determine if further steps are needed to meet expectations of the program.Ensure compliance with regulatory standards.Adhere to benefit/leave protocols.Demonstrate flexibility to perform direct care duties wherever necessary within the program.
