Shift: 9:00 AM-5:00 PM | 7.00 Hours/Day | 35.00 Hours/Week
Pay - $33/hr - $38/hr.


Purpose of Position:
To oversee and improve efforts throughout various hospital departments and settings to screen patients for unmet social needs and connected them to resources able to address those needs. To serve as a subject matter expert for the hospital on social needs screening and referral.

Under the direction of the Chronic Disease Manager, the Program Manager s duties include:

  • Overseeing and expanding workflows for social needs screening and referral, ensuring the hospital meets and exceeds Clients.
  • Ensuring the resources patients are connected to are effective at addressing unmet social needs, such as by regularly liaising with staff from the hospital departments and community-based organizations patients are referred to, reviewing outcomes data, and identifying new resources.
  • Recruiting, training, and supervising a Health Advocate workforce (volunteers and interns) who will engage patients in real time at the hospital and connect those with unmet social needs to set resources.
  • Training the Health Advocates and other interested staff, through an intensive 3-day training held every four months, which the Program Manager will help plan and facilitate, including arranging the speakers, space, food and presentations by invited speakers.
  • Training and supporting other staff engaged in screening and referral, such as Treat 2 Client nurses.
  • Liaising with Central Office staff charged with SDOH screening and referral to obtain support and assistance for Client s workflow and program.
  • Obtaining and reviewing data to ensure the program is a success, and creating presentations and reports as needed.
  • Other tasks as needed.

Machines, Tools, Equipment Operated:

  • Personal computer (including to access Electronic Medical Records)
  • Client/Fax Machine

Knowledge/Skills:

  • Bachelor s Degree/ Master s Degree
  • Knowledge of public health, health policy, law, community health
  • Supervisory experience
  • Excellent written and verbal communication skills
  • Knowledge or familiarity with Excel, data analysis, presentations, reports
