Join Community

Community Health Network was created by our neighbors, for our neighbors. Over 60 years later, "community" is still the heart of our organization. It means providing our neighbors with the best care possible, backed by state-of-the-art technology. It means getting involved in the communities we serve through volunteer opportunities and benefits initiatives. It means ensuring our dedicated caregivers can learn and grow to stay at the top of their fields and to better serve our patients. And above all, it means exceptional care, simply delivered - and we couldn't do it without you.

Partner with Community Health Network and Deaconess Health System - IHCI

The Innovative Healthcare Collaborative of Indiana LLC (IHCI) is a company formed through the partnership of Community Health Network (CHNw) and Deaconess Health System (DHS). Both CHNw and DHS place high importance on continuing and advancing population health and value-based care to improve patient health outcomes.

Make a Difference

The Innovative Healthcare Collaborative of Indiana LLC (IHCI) is seeking an organized and collaborative Project Coordinator to support Population Health Operations initiatives. Reporting to the Director of Population Health Operations, this role will coordinate projects, facilitate meetings, maintain project documentation, track progress, and support process improvement efforts across the organization.

Key Responsibilities:

  • Coordinate project plans, timelines, deliverables, and documentation for population health and process improvement initiatives.

  • Facilitate project meetings, develop agendas, capture meeting notes, and track action items.

  • Monitor project progress, risks, issues, and milestones while providing regular status updates to stakeholders.

  • Coordinate and manage executive leadership calendars to support project update meetings and related activities.

  • Collaborate with leaders, vendors, and cross-functional teams to support project success.

  • Assist with reporting, presentations, metrics tracking, and project logistics.

  • Support data-driven improvements that enhance population health results and operational effectiveness.

Exceptional Skills and Qualifications

Applicants for this position should be able to collaborate with others in a team setting, have excellent communication skills, and a positive attitude toward problem-solving.

  • A bachelor"s degree in a related field is required.

  • Two (2) years of project coordination or related experience, preferably in healthcare or process improvement environments.

  • Two (2) years of experience managing multiple priorities in a fast-paced setting.

  • Strong organizational, communication, and stakeholder engagement skills.

  • Experience with Microsoft Office applications, including Excel, PowerPoint, and Teams.

  • Working knowledge of project management methodologies such as Agile, Scrum, Lean, or Waterfall.

  • Project Management Professional (PMP), Lean Six Sigma, Scrum Master, Agile Certified Practitioner, or similar certification preferred.

  • Healthcare, population health, or operational improvement experience preferred.

  • Ability to build relationships across all levels of the organization.

  • Self-motivated, adaptable, and comfortable managing multiple projects simultaneously.

  • Passionate about improving processes and supporting organizational initiatives.

  • This position will allow flexibility to work remotely. Community caregivers performing work remotely are permitted to live in the following states: Indiana, Illinois, Ohio, Michigan, Kentucky, Florida and Texas. Caregivers are not allowed to perform work remotely outside of the above states. Applicants from other states may apply; however, if hired, they will be required to relocate to one of the above states within 60 days of their employment date.

Why Community?

At Community Health Network, we build teams that deliver exceptional care through empathy, communication and collaboration. We consider ALL an integral part of the exceptional patient experience. We PRIIDE ourselves on not having employees but Caregivers. Join our Community as we make a difference in your community.
