    About the Opportunity

    Guardian Home Care is seeking an experienced healthcare professional with established relationships within Michigan Medicaid, managed care organizations (MCOs), provider networks, or long-term services and supports (LTSS).

    This role is ideal for candidates currently working in Provider Relations, Network Development, Case Management, Care Coordination, Community Relations, LTSS, Waiver Programs, or Managed Care who are interested in joining a rapidly growing home care organization and helping expand referral partnerships throughout Michigan.

    We are particularly interested in candidates with experience working for or alongside organizations such as UnitedHealthcare, Meridian, Molina Healthcare, or similar Medicaid-focused healthcare organizations.

    Position Overview

    The Provider Relations & Medicaid Network Development Manager will be responsible for building relationships with referral sources, managed care organizations, waiver programs, case managers, discharge planners, and community partners throughout Michigan.

    This position combines office-based operational responsibilities with community outreach and relationship development activities. The individual will work closely with leadership to expand referral partnerships, strengthen managed care relationships, and support Our Home Care's continued growth throughout Michigan.

    Key Responsibilities

    • Develop and maintain relationships with Medicaid managed care organizations, provider networks, and referral partners.

    • Serve as a liaison between Our Home Care and healthcare organizations throughout Michigan.

    • Identify new referral opportunities and growth initiatives.

    • Meet with case managers, care coordinators, social workers, discharge planners, and community agencies.

    • Support contract growth and network expansion efforts.

    • Maintain knowledge of Michigan Medicaid programs, waiver services, LTSS, and managed care initiatives.

    • Assist leadership with strategic planning and market development activities.

    Preferred Background

    Experience in one or more of the following:

    • LTSS (Long-Term Services and Supports)

    • Waiver Programs

    • Case Management

    • Care Coordination

    • Healthcare Business Development

    Experience with organizations such as:

    • UnitedHealthcare

    • Meridian

    • Molina Healthcare

    • AmeriHealth Caritas

    • Aetna Better Health

    • Priority Health

    • Other Medicaid Managed Care Organizations

    Qualifications

    • Healthcare industry experience required.

    • Experience working with Medicaid populations strongly preferred.

    • Existing relationships within Michigan healthcare networks preferred.

    • Strong communication and relationship-building skills.

    • Ability to work independently and manage priorities effectively.

    • Ability to travel locally throughout Michigan for meetings, networking, and referral development activities.

    • Comfortable working both in an office environment and in the community with healthcare partners.

    • Valid driver's license and reliable transportation.
