    We are inviting a compassionate and motivated Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our growing clinical team. We proudly deliver high-quality behavioral health services to residents in nursing homes and assisted living facilities throughout Long Island, NY. This position is ideal for a PMHNP who is passionate about geriatric mental health and dedicated to making a meaningful impact on the lives of older adults.

    Requirements

    Key Responsibilities

    • Perform thorough psychiatric evaluations and ongoing mental health assessments for residents in long-term care settings.

    • Formulate tailored treatment plans that reflect each patient’s specific mental health needs and diagnoses.

    • Oversee and adjust medication regimens based on clinical response and best-practice guidelines.

    • Partner with interdisciplinary professionals—including physicians, therapists, social workers, and nursing staff—to ensure coordinated and holistic patient care.

    • Deliver counseling and psychotherapy to residents and support families in understanding care plans.

    • Provide education to patients and caregivers regarding mental health conditions, available treatments, and ongoing self-care strategies.

    • Maintain compliant, accurate, and timely documentation within electronic health records.

    • Engage in case conferences, team meetings, and quality improvement efforts to support clinical excellence.


    Qualifications
    • Active New York State Nurse Practitioner license with certification as a PMHNP.

    • Ability to work independently in a fast-paced clinical environment and manage a diverse caseload.

    • Strong skills in psychiatric diagnosis, treatment planning, psychopharmacology, and patient assessment.

    • Excellent communication and relationship-building abilities with patients, families, and colleagues.

    • Proficiency in EHR systems and general computer use.

    • A compassionate, patient-centered approach with a genuine commitment to geriatric care.
