We’re looking for a compassionate and dedicated <\/span><\/span><\/span>Psychiatric Mental Health Nurse Practitioner (PMHNP)<\/span><\/span><\/span> to join a patient -centered organization committed to transforming mental health care for older adults in skilled nursing and long -term care environments.<\/span><\/span><\/span>

In this role, you’ll provide psychiatric evaluation, treatment, and follow -up care for residents while enjoying a balance of professional independence and the support of an experienced interdisciplinary team. <\/p>
Conduct comprehensive psychiatric evaluations and ongoing follow -up visits.<\/span><\/span>
<\/span><\/p><\/li>
Provide patient -centered care that is trauma -informed, empathetic, and culturally competent.<\/span><\/span>
<\/span><\/span><\/p><\/li>
Develop, prescribe, and adjust individualized treatment plans and medication regimens.<\/span>
<\/span><\/span><\/p><\/li>
Communicate with families, caregivers, and facility staff to ensure a shared understanding of patient care goals.<\/span>
<\/span><\/span><\/p><\/li>
Complete detailed, compliant clinical documentation within required timeframes.
<\/span><\/span><\/p><\/li><\/ul>
Current <\/span><\/span>PMHNP certification<\/span><\/span> and unrestricted <\/span><\/span>state license<\/span><\/span> to practice as a Nurse Practitioner.<\/span>

Excellent diagnostic, assessment, and communication skills.<\/span>
<\/span><\/span><\/p><\/li>
Experience in geriatric psychiatry or long -term care is preferred but not mandatory.<\/span>
<\/span><\/span><\/p><\/li>
Highly organized, proactive, and able to manage a flexible caseload.<\/span>
