Job Description

At Heartland Family Service, we are committed to building a culture that is trauma informed and values and celebrates diversity and inclusion. We believe this allows for better service delivery and innovation, as it encourages our employees to bring various experiences and uniqueness to the workplace.

Trauma Informed Care is an approach to engaging people with histories of trauma that recognizes the presence of trauma symptoms and acknowledges the role that trauma has played in their lives.  Trauma Informed Care also acknowledges the effects of working with trauma survivors on our workforce and seeks to build collaborative and supportive working environments and relationships.

If you believe in our mission of creating healthy families and communities, and like a fast pace, collaborative and team-oriented environment, then Heartland Family Service is the agency for you.

Founded in Omaha in 1875, Heartland Family Service strengthens individuals, families, and communities through advocacy, education, counseling, and support services. Last year we connected with roughly 153,000 people across 20 locations in east central Nebraska and southwest Iowa. Our vision is simple and ambitious: Strong Communities, So Everyone Can.

We're committed to a culture that is trauma-informed and that values and celebrates diversity and inclusion — because when people bring their whole selves to work, we deliver better care and better ideas. Trauma-Informed Care shapes how we engage the people we serve and how we support one another as a workforce.

If you believe in creating healthy families and communities and thrive in a fast-paced, collaborative, team-oriented environment, Heartland Family Service is the agency for you.

SUMMARY DESCRIPTION  OF  WORK

Be the calm in someone's hardest moment. At The Crisis Center, people walk through our doors on some of the worst days of their lives — and walk out steadier, safer, and pointed toward hope. As our full-time Psychiatric Nurse Practitioner, you're the clinical expert who makes that turnaround possible.

This is voluntary, community-based crisis care — not a hospital ward and not a revolving door. It's a warm, respite-style environment where your evaluations, your prescribing, and your clinical judgment directly shape whether someone stabilizes close to home or has to be sent somewhere far away. If you want your expertise to matter on a human scale, this is the room to be in.

You'll plan, implement, and evaluate programs of nursing care and therapeutic intervention that support stabilization, promote wellness, and help people reintegrate into the community — always in step with the Nebraska Nurse Practice Act, Nebraska DHHS behavioral health regulations, and facility policies.

Compensation: Salary is determined by total years of relevant experience.
Work Schedule: 40 hours per week

Essential Duties and Responsibilities

Assess & deliver nursing care

  • Provide comprehensive psychiatric nursing assessments — routine and urgent/emergent as clinically indicated.
  • Conduct physical health screenings, assess ongoing medical needs, and screen for health risks at intake and periodically.
  • Identify conditions requiring referral, consultation, or transfer to a higher level of care.
  • Plan, implement, and evaluate individualized nursing care plans consistent with the interdisciplinary treatment plan.

Manage medications

  • Administer and manage medications in collaboration with the CSRC psychiatric provider and other licensed providers, per the Nebraska Nurse Practice Act and orders.
  • Perform medication reconciliation at admission and discharge, and monitor therapeutic response, side effects, and effectiveness.
  • Ensure secure storage and accurate documentation of medications, including controlled substances.
  • Provide medication education to individuals and families.

Monitor health & withdrawal risk

  • Monitor vital signs and overall physical health status.
  • Watch for indicators of substance use complications or mild withdrawal. The program is not a detox facility — individuals needing medical detox are referred or transferred per policy.
  • Provide crisis health intervention within scope of practice, and implement preventive and remedial wellness plans.
  • Coordinate referrals for external medical services when indicated.

Stabilize the crisis

  • Provide crisis health and psychiatric intervention to support stabilization.
  • Use approved de-escalation techniques and the least restrictive interventions.
  • Participate in ongoing risk assessment, safety planning, and multi-agency service coordination grounded in best practice.

Support people & families

  • Provide therapeutic nursing interventions to individuals and families.
  • Teach education groups on medication management and basic health care, and offer health coaching and wellness activities.
  • Support individuals and families through stabilization and reintegration, and help build sustainable health and recovery plans.

Connect & collaborate

  • Work closely with the interdisciplinary team and key community agencies.
  • Coordinate and schedule follow-up care — medical, mental health, and community-based support.
  • Cross-train teammates in health and wellness interventions, and educate staff on physical and behavioral health issues.

Document, comply & show up well

  • Maintain accurate, timely documentation — assessments, care plans, progress notes, medication records, and discharge summaries that reflect clinical decision-making and coordination of care.
  • Stay compliant with Nebraska DHHS regulations, HIPAA, and agency policies.
  • Be dependable and punctual, collaborate across the agency, take part in quality improvement, and treat everyone with courtesy, respect, and care.
  • Perform other program-related duties as assigned.
