    To engage members in the work of the Clubhouse within a designated work unit. To work collaboratively with Clubhouse members, work unit staff partners, and the Wellspring community overall to maintain a full and vibrant work ordered day. To facilitate member skill and confidence building through the work of the Clubhouse, and to work directly with individual members in skill building and resource connection toward goal attainment in their desired goal areas.

    JOB DUTIES AND RESPONSIBILITIES:

    Rehabilitation:

    • Work collaboratively with unit colleagues (member and staff) to complete the essential functions of the work unit.

    • Establish positive relationships with members.

    • Engage/motivate members in work necessary to run the unit.

    • Create opportunities for member involvement and input, leadership, learning and working.

    • Identify member strengths and special needs.

    • Establish realistic goals for the unit.

    • Work together with members to establish realistic personal and/or rehabilitation goals.

    • Identify individual member learning styles and develop appropriate training and skill building methods.

    • Show sensitivity to member issues and difficulties.

    • Provide support, guidance, direction understanding, etc.

    • Anticipate problems and difficulties regarding members and unit functioning.

    • Effectively intervene and resolve crisis situations.

    • Create opportunities for members to work together.

    • Train members in good work habits and attitudes.

    • Reach out to absent members through phone calls, email, cards, letters, visits.

    • Complete required rehabilitation paperwork.

    • Demonstrate knowledge of mental illness, symptoms, symptom management, coping skills, medications, etc.

    • Address drug and alcohol issues when necessary.

    Supported and Transitional Employment:

    • Provide support for vocational development for individual members, including job readiness, job development, job coaching, and maintaining ongoing support.

    • Within the Transitional Employment program, work as a placement manager in partnership with another staff, including learning job skills and duties, training members on the job, providing on-going support, and filling in when a member is absent.

    • Work in collaboration with placement manager partners and Employment Coordinator to ensure ongoing and positive communication with employer in Transitional employment.

    • Complete ACRE training for provision of supported employment services.

    • Facilitate referrals and maintain ongoing, accurate, and timely reporting for members involved in formalized supported employment through OVR.

    Case Management:

    • Become informed of local services/resources.

    • Advocate for members in need of services.

    • Teach members how to access services/resources.

    • Coordinate services and collaborate with other providers.

    • Communicate effectively with other providers.

    Social/Recreational:

    • Plan and carry out social/recreational activities.

    • Be flexible in work schedule to allow for some evening, weekend, and holiday social/recreational events.

    Team:

    • Communicate effectively and often with supervisor and co-workers.

    • Participate actively in team process, sharing ideas, opinions, giving feedback and support.

    • Participate in joint activities.

    • Ensure adequate staff coverage.

    • Take additional responsibilities when necessary.

    Driving:

    • Drive safely and report vehicle problems.

    • Share driving responsibilities by transporting members in van for shopping and social/recreational activities.

    Additional Job Duties and Responsibilities:

    • Maintains confidentiality of all materials handled within the Network/ Entity as well as the proper release of information.

    • Complies with Network and departmental policies regarding issues of employee, patient and environmental safety and follows appropriate reporting requirements. Demonstrates/models the Network's PCRAFT values during interactions with all customers.

    • Demonstrates Performance Improvement in the following areas as appropriate: Clinical Care/Outcomes, Customer/Service Improvement, Operational System/Process, and Safety.

    • Demonstrates financial responsibility and accountability through the effective and efficient use of resources in daily procedures, processes and practices.

    • Complies with Network and departmental policies regarding attendance and dress code.

    • Demonstrates competency in the assessment, range of treatment, knowledge of growth and development and communication appropriate to the age of the patient treated.

    PHYSICAL AND SENSORY REQUIREMENTS:

    Sit up to 2 hours per day; 1 hour at a time. Stand for up to 10 hours per day; 8 hours at a time. Walk 6 hours per day; 10 minutes at a time. Consistently lift, carry and push objects up to 10 lbs. Occasionally lift, carry and push objects up to 75 lbs. Transports patients weighing up to 500 lbs. via wheelchair, bed and/or stretcher. Frequently stoop and bend. Frequently reach above should level. Must be able to perceive attributes of an object through touch. Must be able to hear as it relates to normal conversation and high and low frequencies. Must be able to see as it relates to general, near, far, color and peripheral vision.

    EDUCATION:

    Bachelor's Degree with academic concentration in an area relevant to the position preferred or; An Associate's Degree and 1 year of work experience in mental health direct service or; A CPS certificate and one additional year of paid or volunteer work experience in mental health direct service or; High School Diploma or equivalency and two years' work experience in human services, which must include one year of mental health direct care experience.

    TRAINING and EXPERIENCE:

    A commitment to working with individuals with longer term mental illness. Ability to teach skills individually and in small groups, link individuals to community resources, provide supportive counseling, and manage emergent issues. Excellent communication and organizational skills. Must have and maintain a valid driver's license accepted by the Network's insurance carrier and maintain a good driving record.
