    Description

    Purpose:
    · To provide Psychiatric Rehabilitation Services (PSR) facility based and/or in the community as assigned
    · To facilitate groups/units to enhance an individual’s knowledge to develop, enhance and retain skills and competencies in living, learning, working and socializing.
    · To help individuals receive the level of and/or have access to the services/supports they choose, so that an individual can live in the environment of their choice.
    · To monitor and track progress of individual’s in assigned caseload; participate in the development of individualized rehabilitation plans.
    Supervision Received:
    · Receives direct supervision from the PRS Director/Team Leader or Psych Rehab Specialist if assigned.
    Supervision Exercised:
    · N/A
    Qualifications:
    · Bachelors degree, or Associates degree and 1 year work experience in mental health direct service, or
    CPS certification and 1 year paid or volunteer experience, or a High School diploma/GED and 2 years
    work experience in Human Services, which must include 1 year of mental health direct service.
    · Must have excellent oral and written communication skills, as well as be detail-oriented and able to
    complete a high volume of documentation in a fast-paced environment.
    · Strong computer skills are required, as well as Internet research abilities.
    · Ability to work independently and as part of a team.
    · Knowledge of mental health system and supports.
    · Must possess a valid driver’s license with an acceptable driving record. (preferred)
    · Must be able to use public transportation to meet with members out in the community or in their homes.
    Scope:
    · Frequent talking and listening.
    · Requires lifting and/or carrying and the ability to fold, kneel, stack, stoop and bend which may include direct client intervention.
    · Must be available for local travel.
    · May require crisis intervention for emergency situations.
    · Participation in on call coverage as applicable.
    · May be required to report to work during emergencies including inclement weather.
