We are seeking a dedicated Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our growing behavioral health team in Nassau County, New York. We partner with nursing homes, rehabilitation centers, and long-term care communities to deliver compassionate, evidence-based psychiatric services to older adults.

Requirements



Key Responsibilities

  • Conduct comprehensive psychiatric evaluations and mental status assessments for residents in skilled nursing, assisted living, and rehabilitation settings.

  • Design, implement, and monitor individualized treatment plans, including psychotropic medication regimens and behavioral interventions.

  • Reassess and adjust care strategies based on clinical progress and patient feedback.

  • Collaborate closely with physicians, psychologists, nurses, and social workers to deliver integrated, holistic care.

  • Maintain accurate, timely, and compliant documentation within the electronic health record (EHR).

  • Contribute to case conferences, facility rounds, and quality improvement initiatives to strengthen care outcomes.


Qualifications


  • Active New York State Nurse Practitioner license and PMHNP certification.

  • Prior experience in adult or geriatric psychiatry preferred (new graduates welcome).

  • Proficiency in psychopharmacology and evidence-based treatment modalities.

  • Strong independent clinical judgment and ability to manage a consistent caseload.

  • Excellent communication and collaboration skills across multidisciplinary teams.

  • Comfortable using EHR systems and digital documentation tools.
