Telos Health Systems is seeking a Psychotherapist (LCSW, LPC, LMFT, PsyD, or PhD) to provide mental health evaluations and psychotherapy services to residents in Skilled Nursing Facilities and Assisted Living Facilities. What you'll do 

  Conduct comprehensive mental health assessments and diagnostic evaluations. 

  Provide individual psychotherapy and ongoing treatment services. 

  Develop, implement, and update individualized treatment plans with the multidisciplinary care team. 

  Maintain accurate, timely, and compliant clinical documentation. 

  Coordinate care with facility staff, medical providers, and other healthcare professionals. 

  Adhere to all federal, state, and organizational standards, policies, and regulations. 


What we're looking for 

  Active Pennsylvania license as an LCSW, LPC, LMFT, or Licensed Psychologist (PsyD/PhD). 

  Master's degree (LCSW, LPC, LMFT) or Doctoral degree (Psychologist) in a relevant mental health field. 

  Ability to work independently while collaborating effectively within a clinical team. 

  Strong communication and clinical documentation skills. 

  Preferred: experience working with geriatric populations. 

  Preferred: experience in Skilled Nursing, Assisted Living, or long-term care settings. 



Compensation & benefits 

W-2 employment with RVU-based compensation and flexible scheduling focused on work-life balance — with no billing or authorization responsibilities (handled by our billing team) and ongoing support from our Directors of Mental Health.
