Page Health & Rehab Center is a premier skilled nursing and rehabilitation facility located in Fort Myers, Florida. Our in-house subacute rehabilitation team is dedicated to providing high-quality, patient-centered care that supports recovery, mobility, and improved quality of life.
We are currently seeking a compassionate and motivated Full-Time Physical Therapist (PT) to join our collaborative rehab team.
Sign-On Bonus
A sign-on bonus of up to $10,000 is available for qualified new full-time Physical Therapists.
Physical Therapist Position Summary
The Physical Therapist evaluates residents’ physical abilities, develops individualized treatment plans, and delivers evidence-based therapeutic interventions to restore mobility, strength, balance, and functional independence. This role works closely with an interdisciplinary team to ensure coordinated and effective patient care.
Physical Therapist Key Responsibilities
Evaluate residents’ mobility, strength, balance, endurance, and functional abilities
Develop and implement individualized physical therapy treatment plans
Provide high-quality, evidence-based PT services aligned with patient goals
Collaborate with Occupational Therapists, Speech-Language Pathologists, nursing staff, physicians, and other interdisciplinary team members
Educate residents, families, and caregivers on treatment plans, mobility techniques, and safety strategies
Maintain accurate and timely clinical documentation in compliance with facility and regulatory standards
Participate in care planning meetings, interdisciplinary team conferences, and discharge planning
Adhere to all state, federal, and facility policies, including HIPAA and infection control guidelines
Physical Therapist Qualifications
Graduate of an accredited Physical Therapy program
Current Physical Therapist license in the state of Florida
CPR certification (or willingness to obtain upon hire)
Experience in skilled nursing or subacute rehabilitation preferred, but not required
Strong communication, organizational, and interpersonal skills
Ability to work independently and collaboratively within a multidisciplinary team
