As a PT Home Care provider, you will deliver high-quality, personalized rehabilitation services to patients in the comfort of their own homes, helping them regain mobility, improve function, and enhance their quality of life.
Responsibilities
- Conduct comprehensive patient assessments and develop individualized treatment plans.
- Provide therapeutic interventions and monitor patient progress.
- Educate patients and their families on treatment plans and home exercises.
- Document all patient interactions and maintain accurate medical records.
- Collaborate with interdisciplinary care teams to coordinate patient care.
- Ensure compliance with all regulatory and company policies.
Requirements
- Valid Licensed Physical Therapist credential in the state of Michigan.
- Experience in home health care or outpatient physical therapy preferred.
- Strong communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Reliable transportation to travel to patient homes within the Southeast Michigan Region.
