A Medicare-certified and Pennsylvania state-licensed home health agency is seeking an experienced and dedicated Physical Therapist (PT) to provide home-based therapy services on a PRN / Per Diem basis.
This position offers the opportunity to deliver one-on-one skilled care directly in patients’ homes, helping them regain independence, mobility, and confidence following illness, injury, surgery, or hospitalization. The agency is committed to high-quality, patient-centered care and a collaborative multidisciplinary approach that ensures optimal outcomes and patient satisfaction.
Conduct comprehensive in-home physical therapy evaluations to assess functional limitations, safety risks, and rehabilitation needs.
Develop and implement individualized treatment plans tailored to each patient’s goals and medical condition.
Provide therapy interventions focusing on mobility, strength, balance, gait training, and fall prevention.
Reassess progress regularly and adjust care plans in collaboration with the interdisciplinary team.
Educate patients and caregivers on home exercise programs and safe movement techniques.
Document visits and clinical notes in the agency’s electronic medical record system in a timely manner.
Collaborate effectively with nurses, physicians, and other care team members to ensure coordinated and high-quality care delivery.
Uphold professional and clinical standards in compliance with home health regulations.
Active and unrestricted Physical Therapist (PT) license in Pennsylvania.
Home health experience preferred, but not required — full orientation and support provided.
Strong skills in patient assessment, documentation, and education.
Comfortable working independently in a home-based setting.
Reliable transportation and valid auto insurance required for travel to patient residences.
Employment Type: PRN / Per Diem
Scheduling: Flexible — visits assigned based on territory, availability, and patient needs.
Territory Coverage: Philadelphia, Montgomery, Bucks, Delaware, and Chester Counties.
