PT - Home Health - PRN Bench (Montgomery County)

A Medicare-certified and Pennsylvania state-licensed home health agency is seeking an experienced and dedicated Physical Therapist (PT) to provide home-based therapy services on a PRN / Per Diem basis.

This position offers the opportunity to deliver one-on-one skilled care directly in patients’ homes, helping them regain independence, mobility, and confidence following illness, injury, surgery, or hospitalization. The agency is committed to high-quality, patient-centered care and a collaborative multidisciplinary approach that ensures optimal outcomes and patient satisfaction.

Key Responsibilities

• Conduct comprehensive in-home physical therapy evaluations to assess functional limitations, safety risks, and rehabilitation needs.

• Develop and implement individualized treatment plans tailored to each patient’s goals and medical condition.

• Provide therapy interventions focusing on mobility, strength, balance, gait training, and fall prevention.

• Reassess progress regularly and adjust care plans in collaboration with the interdisciplinary team.

• Educate patients and caregivers on home exercise programs and safe movement techniques.

• Document visits and clinical notes in the agency’s electronic medical record system in a timely manner.

• Collaborate effectively with nurses, physicians, and other care team members to ensure coordinated and high-quality care delivery.

• Uphold professional and clinical standards in compliance with home health regulations.

Qualifications

• Active and unrestricted Physical Therapist (PT) license in Pennsylvania.

• Home health experience preferred, but not required — full orientation and support provided.

• Strong skills in patient assessment, documentation, and education.

• Comfortable working independently in a home-based setting.

• Reliable transportation and valid auto insurance required for travel to patient residences.
