Home Health Physical Therapist (PT) – PRN / Per Diem

Position Summary: Our client is a Medicare‑certified and Pennsylvania state‑licensed home health agency dedicated to delivering individualized, high‑quality skilled care in the comfort of patients’ homes. As an ACHC‑accredited organization, we prioritize patient safety, clinical excellence, and empowering individuals to regain independence following surgery, illness, injury, or hospitalization. We are seeking a compassionate and motivated PRN / Per Diem Physical Therapist to join our growing team and provide home‑based therapy services throughout the greater Philadelphia region.

Key Responsibilities

• Conduct comprehensive physical therapy evaluations in patients’ homes
• Develop and implement individualized treatment plans tailored to patient needs
• Deliver therapeutic interventions focused on mobility, strength, balance, and fall prevention
• Monitor patient progress and adjust care plans accordingly
• Educate patients and caregivers on home exercise programs and safe mobility techniques
• Document all visits and clinical findings in the agency’s EMR system
• Collaborate closely with nurses, physicians, and the interdisciplinary care team to ensure coordinated, high‑quality care

Requirements

• Active Pennsylvania Physical Therapist (PT) license
• Home health experience preferred, but not required
• Strong clinical assessment and patient‑education skills
• Ability to work independently in a home‑based setting
• Reliable transportation for travel between patient visits
