We are seeking a Full-time Physical Therapist to join our team! The person we are looking for enjoys appreciation for their hard work, makes a difference providing quality care and wants to be part of a work family not just another employee. *Coverage area determined by candidates location; however, we are seeking to cover northern Kalamazoo, Allegan, and Barry counties and surrounding areas.

Qualifications:

· Ability to perform therapy evaluations, routine visits, and OASIS documentation

· Work with other disciplines to give the best care needed for each patient.

· Help decrease the need for re-hospitalization by being actively involved in the patient's plan of care to assess any changes that may be needed.

· When appropriate teach and counsel family members about care and processes for disease/injuries
