Hiring Range: $57,346 - $75,266.63
This is an exempt (salaried) position. Pay rates are based on education, skill, experience level and internal equity. Internal equity consideration included an assessment of the applicant's salary history and qualifications in comparison to the market rate and requirement for the job.
This is a Grant Funded Position through December 31, 2026, with renewal options based on available funding.
This role is on-site 5 days a week, 8:00 AM to 5:00 PM.

Position Summary
Responsible for providing professional-level Care management services for various populations. Works
in a variety of settings coordinating care needs across the healthcare continuum. Incumbents will provide
evaluation, care management, counseling, and referral services to assist in meeting the clients'' needs.
Duties include linking and arranging of services and advocacy to implement a person-centered plan.

Minimum Qualifications
Experience:
Minimum of one year of public health, healthcare and/or regulatory experience
Education:
Social Worker: MSW from an accredited school of social work; BSW from an accredited school of social
work and one year of social work experience.
Nurse: BSN from an accredited nursing education program; ADN from an accredited nursing education
program
Combination of relevant education and relevant experience accepted?: N/A
Licenses and Certifications
Heart Association that is valid for at least 90 days after hire.
