Here’s what you’ll be doing:

  1. Assists with the development and implementation of multiple strategies, initiatives and activities for individuals, families and population groups that promote health and prevent disease within the FCP Community.
  2. Assists in the development, implementation and evaluation of strategies that address established community health improvement priorities in partnership with Tribal and Local partners.
  3. Designs and conducts health educational programs for the FCP Community.
  4. Serves as Public Health Accreditation Coordinator and ensures the Community Health Department is meeting all requirements to maintain Public Health Accreditation.
  5. Gathers, organizes and maintains data for program monitoring, evaluation and improvement.
  6. Serves as the Public Health Emergency Preparedness (PHEP) Coordinator, fulfilling PHEP grant deliverables, and maintaining public health emergency preparedness plans, ensuring alignment with tribal, local, state and federal guidelines.
  7. Assists with leading coordination efforts during public health emergencies, including collaboration with internal and external partners to support response and recovery activities.
  8. Organizes, coordinates, and evaluates emergency preparedness training and exercises to ensure staff readiness, and test and improve response plans.
  9. Promotes the community’s understanding of, and advocacy for, policies and activities that will improve the community’s health.
  10. Develops and maintains various departmental plans including Community Health Assessment, Community Health Improvement Plan, Quality Improvement, Performance Management, and Strategic Plan.
  11. Implements health education and youth programing in partnerships with other Forest County Potawatomi entities; completes event evaluations for health education and health promotion events and programs held by the department.
  12. Assists in the departments grant efforts which may include trainings, conferences, and meetings.

Here’s what you’ll need to be successful:

  • Bachelor’s Degree in Public Health, Health Education, Health Promotion or other related field
  • Three (3) years of experience in Public Health or Community based setting required
  • Possess and maintain a valid driver’s license and be eligible for coverage under the Tribe’s Insurance
  • Must successfully pass all applicable background checks and drug screens

 Preferred Qualifications:

  • Certified Health Education Specialist (CHES) preferred
  • Experience with Public Health Accreditation preferred
  • Experience with Emergency Preparedness preferred
