About the Position
This is a Time-Limited position. - Service Coordinator/Coach will have a caseload for service coordination with children (birth to three) with mild to moderate disabilities, accessing health and developmental services and making referrals to licensed providers (Speech, Occupational, Physical, etc.). Will conduct intake by meeting the family in the Natural Environment or at the Clinic. Will develop goals, including the child's needs, strengths, and diagnosis (if given). Plan Individualized Family Plans with parents present and/or other members present at the request of the parent. Met timelines; encourage parent to bill private and/or Medicaid. Provide Targeted Case Management. Facilitate the development of a transition plan. Participate in community events, family needs, and refer to other agencies within the health department. Provide coaching, training, and guidance to EI Service Coordinators on evidence-based practices and document progress on implementation.
What youll need to be Successful
To be successful, individuals in this position must have a commitment to serving a diverse population of families who have children who may have medical and/or developmental delays. They must also know about child development and family-centered practices and have good organizational and leadership skills.
Preferred Qualifications
Typically requires a Bachelor's Degree and 4+ years of experience. - Agency Prefers: At least two (2) years' experience in service coordination for children with disabilities, birth to eighteen years of age, and service provision to children under six.

Paid Personal Leave and Sick Leave
Full-time employees receive paid time off for personal needs and for sick leave. Leave begins accruing after one month of employment and may be used as it is accrued. Employees earn approximately 18 days of personal leave annually and 12 days of sick leave annually.
Military Leave
In accordance with federal law, all employees serving in the armed forces or the military reserves are entitled to 15 days per year for military training.
