Community Healthcare Workers, Public Health Workers, social workers or case managers that have worked within a public healthcare system

To support the VA Boots on the Ground Services for Community Program Support

Summary:

The regions Covered are the continental U.S., the island of Oahu, the rest of the Hawaiian Islands, and the rest of the Pacific.

We are seeking dedicated Community Healthcare Workers to support veteran care initiatives in Hawaii. This vital role involves providing comprehensive support to veterans by assisting with healthcare navigation, patient education, and coordination of services within community and clinical settings. The ideal candidate will play a crucial part in enhancing patient outcomes, ensuring safety, and promoting well-being among veterans through compassionate, culturally sensitive care. This position offers an opportunity to contribute meaningfully to veteran health services while working in a dynamic, multidisciplinary environment.

· Qualifications:

Experience

Demonstrated knowledge of medical terminology, patient safety practices, and infection control procedures.
Background in geriatrics, medical-surgical units, or post-acute care environments is preferred.
Hands-on experience with medical equipment operation, CPR certification, and basic life support (BLS).
Inpatient or outpatient experience within teaching hospitals or sub-acute care facilities is beneficial.
Proven ability to work effectively with diverse patient populations, including geriatric patients.
Strong communication skills for caregiver interaction and interdisciplinary collaboration.
Experience in Veteran healthcare support would be helpful, but not a requirement. 

· Promoting VHA services.

· Assessing Veterans’ unique needs in the continental U.S., Hawaii, and Pacific Islands.

· Connecting Veterans to community and health resources, supporting enrollment, appointment follow-ups, and treatment compliance.

CHW certification is required where applicable.

Responsibilities

Assist veterans with navigating healthcare services, including outpatient clinics, inpatient facilities, and post-acute care settings.
Support clinical staff by helping with patient transfer methods, vital signs monitoring, and basic medical documentation.
Promote patient safety and infection control protocols during interactions and procedures.
Communicate effectively with patients and caregivers to provide education on health management, medication adherence, and preventive care.
Maintain patient confidentiality in compliance with privacy regulations and organizational policies.
Support medical equipment operation and ensure proper handling during routine procedures.
Facilitate communication between patients and healthcare providers to ensure coordinated care delivery.
Assist with infection control measures within care facilities and promote adherence to safety standards.
Document patient interactions accurately within electronic health records (EHR) systems.
Contribute to the training of new staff or interns on clinical workflows and patient interaction protocols.
