IF SELECTED THE FOLLOWING IS REQUIRED: Fingerprint verification at an approximate cost of $64 to the employee; Proof of TB clearance (within 4 years) at employee's cost or a "Certificate of Completion" signed by a licensed health care provider for the Adult TB Risk Assessment Questionnaire; Certificate of Completion for the CA Mandated Reporter Training dated after July 1, 2026.
