RESPONSIBLE TO: Administrator, Alternate Administrator, QAPI Director
MAIN FUNCTION:To review client charts for technical details and completeness to ensure all documentation has been obtained to support claims.

ESSENTIAL TASKS:
- Check that charts are set up correctly.

- Review charts periodically throughout the quarter to maintain the established filing system.

```
- Monitor completeness of required documentation.
```{=html}
```
- Monitor tracking sheets to ensure timely start of care.
```{=html}
```
- Monitor accuracy of billing for services.
```{=html}
```
- Audit EVV(Electronic Visit Verification) reports to ensure accuracy and compliance with EVV system requirements.

```
- Attend and participate in QAPI (Quality Assurance Performance Improvement) Committee meetings
```{=html}
```
- Prepares and submits reports as directed by the Administrator and QAPI Director.

```
- Reports all Agency-related matters requiring action to the Administrator and QAPI Director.

```
- Is able to listen and follow instructions well.

```
- Attends and participates in training and in-services.
```{=html}
```
- Audits case conference sheets
```{=html}
```
- Ensure supervisory visits are scheduled and completed timely
```{=html}
```
- Double check all authorizations put in by PHC.
```{=html}
```
- Travel to Branch office(s) to review charts
```{=html}
```
- Other duties as assigned by the Administrator and QAPI Director.
MINIMUM TOOLS AND EQUIPMENT REQUIRED:
- Computer, calculator, copier/fax
EDUCATION AND EXPERIENCE:
- High School graduate or equivalent.

- Demonstrates a working knowledge of State and Federal Regulations and HHSC and various MCO contract rules.
SKILLS AND ABILITIES:
- Able to lift or move up to 25 pounds.

- Able to sit for 6-8 hours per day.

- Ability to carry out varied job duties with little supervision.

- Is able to assess notes related to billable issues, Federal or State Guidelines.
