Position Summary: Oversees increasing overall performance by enhancing the managing, planning and training for all key operations and assessing the outcomes to fulfill or exceed CMS regulatory requirements.
Critical Tasks:
Assists in the development of clinical outcome measures, related tracking and monitoring systems and a mechanism to integrate efforts into organization-wide critical pathways (CMS), patient safety, compliance, and care quality systems.
Coordinates regulatory implementation of Performance Improvement Plans, Plans of Correction, and QAPI meeting follow up.
Participates actively in a variety of committees providing information, reports and recommendations relevant to care quality and resident safety.
Works closely and collaboratively with all clinical and nonclinical skilled nursing facility staff.
Collaborates with other departments to promote process improvement and outcome improvement strategies.
Acts a business partner with GRHC to support initiatives and goals.
Provides support as needed in quality improvement programs/initiatives as indicated through the PIP and QAPI process.
Participates in a variety of skilled nursing facility/long term care educational programs to maintain current skill and competency levels; identifies and discusses performance or training needs with Administrator, Director of Nursing, and reports to QAPI committee.
Acts as a resource to Coordinator of Staff Development and provides backup support as needed.
Communicates areas of concern timely to Director or Nursing and Administrator.
Participates in the development of plans of correction for any deficiencies identified during surveys.
Participates as an active member of The Caring House's Quality Assurance and Performance Improvement (QAPI) Committee.
Analyzes quality and regulatory reporting metrics to improve resident outcomes including the CASPER.
Utilizes various metrics to evaluate skilled nursing facility objectives to achieve successful outcomes.
Participates in GRHC Quality Council Meetings.
Performs other duties or special projects as assigned.
Required Qualifications:
Graduate of a Registered Nurse program from an accredited university or college, Bachelor's Degree preferred.
Registered Nurse licensure in Arizona in good standing or qualified Compact Licensure, and current BLS certification.
Required quality manager certification within two years, QAPI Certified Professional preferred.
Employment at the Caring House requires AZ Department of Public Safety Fingerprint Clearance Card.
2 years of nursing experience with previous experience in SNF, long term care setting.
Previous experience as quality manager, or equivalent desired.
Previous management experience; preferred.
Must possess proficient knowledge of the Federal CMS rules and regulations.
Demonstrated ability to relate to diverse cultures and specifically the Gila River Community and/or other Native American cultures and community health services.
