Case Manager provides evaluations, consultation and intervention with youth and families within La Vega ISD and in the community. Also participates in conducting psychosocial programming with this population as part of a clinical team.
EXAMPLES OF WORK PERFORMED:- Works with youth and their families in educational, mental health, home or office settings.
- Works with school personnel and community providers for the purpose of creating and maintaining individualized service arrays for families.
- Provides case management and some skills training for children and families.
- Make assessment and psychosocial evaluations and recommendations.
- Refer families of consumers to appropriate social welfare and health agencies to obtain assistance.
- Performs related work as required.
Experience:
Experience as set forth under Education. Must have current valid Texas driver's license and current and continued proof of liability insurance.
Education:
Minimum bachelor's degree in psychology, social work or related fields. Must be a licensure candidate (LPCi, LMSW, etc.)
KNOWLEDGE, SKILLS, AND ABILITIES- Knowledge of emotional problems and treatment approaches.
- Ability to establish and maintain positive working relationships with consumers and colleagues.
- Good work habits and organizational skills.
- Familiarity with computer operation.
- Must have excellent written/verbal communication.
- Must be able to travel locally 5% and regionally 30% and transport clients.
- Must be able to lift up to 156lbs.
- Must have ability to do clinical documentation accurately and timely.
- Must have assessment/diagnostic eval skills, and rehabilitation/skills training.
- Must have and maintain a level of general health that permits the performance of the job duties and ability to maintain regular attendance as set forth in Center policies and procedures.
- Level of general health permitting stooping, bending, climbing stairs and participating in moderately active recreational activities.
- Physical ability to perform CPR and PMAB skills.
