Case Manager provides evaluations, consultation and intervention with youth and families within the ISD in which they are working and in the community. Also participates in conducting psychosocial programming with this population as part of a clinical team.
EXAMPLES OF WORK PERFORMED: - Works with youth and their families in educational, mental health, home or office settings.
- Works with school personnel and community providers for the purpose of creating and maintaining individualized service arrays for families.
- Provides case management and some skills training for children and families.
- Make assessment and psychosocial evaluations and recommendations.
- Refer families of consumers to appropriate social welfare and health agencies to obtain assistance.
- Performs related work as required.
Experience, Education and Training- Minimum bachelor's degree in psychology, social work or related fields.
- One year case management Preferred
- Must have current valid Texas driver's license and current and continued proof of liability insurance.
KNOWLEDGE, SKILLS AND ABILITIES:- Considerable knowledge of the principles, methods, techniques and practices of therapy and social work, the functioning of social service or treatment facility of community resources and how to use them effectively.
- Must have and maintain a level of general health which permits the performance of job duties.
- Must have excellent verbal/written communication skills.
- Must have excellent time management and organizational skills.
- Must be able to transport clients.
- Must have excellent clinical documentation skills.
- Must have basic computer skills.
- Ability to lift up to 30lbs.
Hours: Mon-Fri 8am-5pm
Salary: $21.6346-25.4639 Hourly $45,000-$52,965 Annually **Depending on education & experience**
