Qualified Intellectual Disabilities Professional-QIDP/Case Manager-CLR

**Job Summary:**

Ray Graham Association is seeking a Qualified Intellectual Disabilities Professional (QIDP) to serve as a Case Manager. The QIDP will be responsible for coordinating and overseeing the delivery of services to individuals with Intellectual and Developmental Disabilities, focusing on achieving their personal goals and aspirations.

**Responsibilities:**

- Act as the central point of contact for families, guardians, and agencies involved in the care of individuals with disabilities.
- Establish and nurture relationships to facilitate the provision of necessary services.
- Collaborate with healthcare providers to ensure the highest quality of care.
- Conduct and assess evaluations to determine individual outcomes.
- Develop personalized plans and support individuals in pursuing their objectives and aspirations.

**Qualifications:**

- Bachelor's degree in a Human Service field as required by the Illinois Department of Human Services.
- Minimum of one year of direct experience working with individuals with developmental disabilities.
- Strong verbal and written communication skills.
- Clear State Police history and Health Care Worker Registry background check.
- Possession of a valid Illinois driver's license.
- Successful completion of a pre-employment drug screening.
- Ability to work independently and collaboratively in a fast-paced environment.

**Starting Salary:**

$21.18 per hour.
