Role Overview


The QP oversees service delivery, ensures documentation meets regulatory standards, supervises paraprofessionals, and monitors client progress according to the PCP/ISP. The QP ensures high-quality services that meet Medicaid, state, and agency requirements.


Core Responsibilities


1. Clinical Oversight


  • Review and monitor PCPs/ISPs for accuracy and compliance.

  • Ensure services match the goals, interventions, and authorized hours in the PCP/ISP.

  • Adjust plans or request updates when client needs change.

2. Staff Supervision


  • Conduct monthly supervision with paraprofessionals (minimum 1 hour).

  • Review documentation, interventions, and staff performance.

  • Provide coaching and training to improve skill-building, engagement, and compliance.

  • Address performance concerns and follow agency protocols.

3. Client Monitoring


  • Complete required monthly client check-ins.

  • Track progress on goals, outcomes, and barriers.

  • Verify that the client receives services according to the plan and schedule.

  • Ensure client rights, safety, and dignity are always protected.

4. Documentation & Compliance


  • Approve or review staff notes, correcting errors before submission.

  • Ensure all notes include:

    • intervention

    • goal targeted

    • duration

    • effectiveness

    • staff signature

  • Submit timely QP documentation, supervision notes, and monthly reports.

  • Ensure all services follow NC Medicaid, 1915(i), and agency standards.

5. Coordination of Care


  • Communicate with care managers, guardians, natural supports, and other providers.

  • Attend treatment planning meetings, revisions, and interdisciplinary team meetings.

  • Document all coordination of care contacts.

6. Training & Support


  • Ensure paraprofessionals complete mandatory training and remain competent.

  • Provide guidance on crisis response, documentation, service delivery, and ethics.

  • Assist staff with understanding service definitions (ITS, CLS, etc.).

7. Quality Assurance


  • Participate in audits, monitoring reviews, and internal quality improvement.

  • Identify risks, gaps, or errors in service delivery and correct them promptly.

  • Ensure service authorizations, consents, and required forms remain current.

Essential Tasks


  • Monthly supervision with each staff

  • Monthly client contact and monthly summary

  • Review and approve staff documentation

  • Update service plans or request changes when needed

  • Respond to incidents, reports, or concerns

  • Ensure staff follow person-centered practices

  • Participate in meetings, training, and agency updates

  • Provide clinical support and problem-solving strategies to staff

Qualifications


  • Bachelor’s or Master’s degree (Human Services field or equivalent per NC Medicaid rule)

  • Minimum 2 years of experience with MH/I/DD/SE/SED/SPMI population

  • Knowledge of NC service definitions, 1915(i), and Medicaid requirements

  • Clean background check, HCP registry, and OIG list

  • Valid driver’s license and reliable transportation

  • Strong documentation and communication skills

  • Timely and accurate notes, supervision, and reports

  • Documented improvement in staff performance and client outcomes

Physical & Work Requirements


  • Ability to travel to client homes, community locations, and agency office

  • Able to complete in-person and telehealth visits

  • Occasional lifting up to 25 lbs

  • Flexible scheduling depending on client and staff needs
