Job Details
Interprets physicians'' prescriptions and dispenses medications for high dollar specialty and other medications within regulatory, accreditation, and policy guidelines. Provides medication therapy management by actively managing drug therapy; identifies, prevents and resolves medication-related problems. (e.g. Counsels patients and/or medical staff and handles problem resolution). Completes patient records, initial assessments, medication profiles, initial fill notes and continuing progress notes. Participates in the coordination of care to meet patient outcomes and goals including, initial referral, medication review and selection, assessment of medication delivery system, coordinated care conferences, patient care plans, compliance programs and discharge of patients from services.
Interacts with internal and external clinical and non-clinical personnel to support benefits investigation, financial assistance and prior authorization needs. Supports activities for mitigating patient side effects and promoting therapy adherence. Engages in practice pull through for specialty and other orders. Aids in development and maintenance of patient management program. Participates and undertakes initiatives and projects impacting critical clinical, operational and financial areas.
