Job Duties:
- Triage - Coordinate with providers and nurses to resolve health concerns
- Integrated Care - Promote the delivery of care across the continuum of services
- Care Gap - attainment of identified preventive and chronic disease gaps in care
- Failed Appointments - Follow-up on cancelled/no show medical, dental appointments
- Lab tracking - Monitor lab orders for medical services
- Data - Prepare data on Referrals
Qualifications/Skills:
- Current RN or LPN license in good standing with the State of Louisiana (Required)
- Current Basic Life Support (BLS) (Required)
- Prefer experience in ambulatory care setting

Starting Salary Range:
- LPN - $18.66 - $24.35 per hour; up to 3% annual increases
- RN - $50,230 - $71,240 per year; up to 3% annual increases
