    Description

    Sage Group HealthCare Staffing is seeking a Home Health LPN for a 13-week travel/contract position in Everett, WA Accepting both local and traveler candidates.

    Position Details:

    Position Name: Home Health LPN 

    Location: Everett, Washington

    Specialty: Home Health

    Start Date: ASAP

    Employment Type: Full-time, Travel, Contract, Temporary

    Contract Length: 13-week contract 

    Pay Rate: Range $1680 to $1720 Weekly Gross pay based on a 40-hour work week (Includes non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)

    Mileage Reimbursement: $0.54/mile for distance covered between patient home appointments

    Unit: Home Health Hospice 

    Shifts available: Days: 8 Hours;

    Shift timings: Days, 8a – 5p

    Schedule: 40 hours/week guaranteed, 5 shifts per week, Mon-Fri, Productivity expectation is 30 points per week

    Position Requirements:

    • LPN experience: 1 - 2 years - Required
    • Recent/Current LPN Home Health experience: 1 year within the last 3 years – Required
    • Home Care Home Base (HCHB) charting - Required 
    • Must have valid Driver’s License and Personal Auto Insurance

    Education, License and Certification Requirements:

    • LPN License for State of the state applying or Compact if acceptable - Required
    • BLS Certification from AHA or American Red Cross - Required

    Job Description:

    • Provide skilled nursing care as ordered by the attending physician under RN's supervision.
    • Observe and monitor patient conditions and notify RN case manager of any changes in the patient’s condition.
    • Administer medication as prescribed by the physician under RN's supervision and help decrease re-hospitalizations by educating the patient on preventative measures and making good decisions.
    • Educate the patient and family on the disease process using teach back methods to ensure patient and family understanding.
    • Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.
    • Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.
    • Attend IDT Team meetings
    • Territories to cover: Snohomish County Ministry (Mostly Everett area and south of that, must be available for 50 mile radius from the office)
