Southeast Healthcare is a mission-driven nonprofit dedicated to supporting individuals with mental health challenges as they reintegrate into the community after involvement with the criminal justice system. We believe in second chances, trauma-informed care, and person-centered planning. We are seeking a compassionate and organized Case Manager to support adults living with serious mental illness who also have a history of criminal justice involvement.
Responsibilities:
- Provide community-based services to support patients in managing severe and persistent mental illness.
- Develop and connect patients to resources that aid in their recovery, such as housing, healthcare, and employment.
- Advocate for clients' access to benefits, treatment, housing, and employment opportunities.
- Promote and encourage healthy lifestyle choices, focusing on both physical and mental health.
- Offer crisis intervention in emergencies to de-escalate situations and ensure patient safety.
- Work collaboratively with the interdisciplinary team to create and implement individualized treatment plans to meet specific patient goals.
- Monitor patient progress and adjust treatment plans as necessary to ensure successful goal achievement.
- Maintain monthly productivity expectation and up-to-date records of patient progress, treatment plans, and interactions.
- Minimum of AA/BA required; degree in social work, psychology or criminal justice preferred.
- Experience working with adults with mental illness and/or criminal justice involvement required.
- Strong interpersonal and communication skills.
- Ability to work effectively as part of an interdisciplinary team.
- Crisis intervention and de-escalation skills.
- Knowledge of community resources and healthcare services.
- Experience with trauma-informed care and harm reduction preferred.
- Familiarity with HIPAA, Medicaid documentation, and local behavioral health regulations preferred.
- Valid driver's license and insurance.
