Community Habilitation Specialist

Schedule

Monday-Friday | 9am-3:30pm | 30 hours total

Minimum Qualifications

Education: High School Diploma or GED required; Associate’s Degree preferred.

Experience: Minimum 1 year of experience working with individuals with intellectual and developmental disabilities (I/DD).

Certifications/Licenses: Valid NYS Driver’s License required; CPR, First Aid, and SCIP‑R certification or ability to become certified within 6 months.

Work Eligibility: Must be authorized to work in the U.S.

Work Location Requirement: Community-based/onsite role.

Compensation

Hourly rate: $19.8077

About the Role

The Community Habilitation Specialist supports individuals with intellectual and developmental disabilities in developing life skills, community engagement, social connections, and personal independence. This role works one‑on‑one with individuals in their homes and community settings to help them achieve goals outlined in their Community Habilitation Plan and Individualized Service Plan (ISP).

Primary Responsibilities

- Provide transportation or accompany individuals to and from community activities.

- Follow goals and activities outlined in the Individualized Service Plan and Community Habilitation Plan.

- Document daily activities, progress, and observations in required service notes.

- Support individuals in developing social skills, recreational involvement, and community relationships.

- Assist individuals and families in identifying skills to build and develop plans to reach those goals.

- Help individuals access social, recreational, and community resources.

- Identify risks for social isolation and develop plans to enhance social connectedness.

- Negotiate schedules and daily activities with individuals and families.

- Submit accurate and timely service documentation and hours worked.

- Follow all action plans and behavioral plans as required.

- Apply knowledge of emergency procedures as outlined by the agency.

- Promote community participation, independence, and positive social interaction.

- Serve as a positive role model.

- Report incidents, concerns, and updates to the Residence Manager.

- Maintain CPR, First Aid, and SCIP‑R certifications.

- Perform other duties as assigned.
