The Patient Navigator Clinical identifies and implements nursing interventions and evaluates results for a wide variety of patient populations, directs and acts as resource to other members of the nursing team as assigned and promotes compliance and support for policies. The primary responsibilities for this position consist of providing comprehensive review of specialty referral which includes: assessing, documenting, planning of referral determination; ensuring patient satisfaction; training and delegating duties; and adhering to University policies and procedures as well as federal, state and local regulations.
Autonomous nursing care is the ability of a nurse to assess and provide nursing actions as appropriate for patient care populations based on competence, professional expertise, and knowledge. The nurse is expected to practice autonomously consistent with ANA Standards of Practice and Professional Performance. Independent judgment is expected to be exercised within the context of interdisciplinary and multidisciplinary approaches to patient care.
Applies knowledge, experience and judgment to determine the importance of a situation, set priorities, and use abstract thought to evaluate clinical situations. Participates in patient and staff education. Demonstrates leadership skills. Demonstrates accountability for own practices as defined by the Nurse Practice Act.

Department Overview
The UCDH Patient Contact Center (PCC) is the front door for patients and families seeking medical care to heal themselves and their loved ones. Our role is to make access to care easy and timely. PCC is a coordinated system of people, processes, and technologies providing access for the Patient, Client, and Organization. We engage with our patients and partners in a compassionate manner so as to enhance the overall image and reputation of UC Davis Health.
Position works with various UCD musculoskeletal, surgical, and specialty clinics. Position may spend time at various clinics as needed for training and staff collaboration. The Patient Navigator Clinical position will support the review of specialty referrals for UCDH's musculoskeletal, surgical, and specialty clinics. The position may be assigned to additional ambulatory specialty clinics to support. Referrals will be assessed by diagnosis, referral notes, imaging, and other diagnostic tests to determine referral and scheduling pathways. Communicate effectively with team members to ensure accurate and timely referral processing. The Patient Navigator Clinical is actively involved in assessing, planning, intervening, educating, and coordinating care for referred patients to our specialty clinics including applying nursing judgement and need to escalate the patient to an urgent level of care.
The Patient Navigator Clinical is a highly skilled individual using protocol-driven care to disposition patients to the correct specialist at the right time, communicate effectively with physicians to clarify referral needs, ensures accurate disposition, seamless handoff to scheduling teams with clear instructions regarding provider and appointment type and exercising sound judgment within interdisciplinary and multidisciplinary care teams. The primary responsibilities for this position includes providing a thorough assessment of available diagnostic information to appropriately direct referrals; utilizing a strong understanding of various complex conditions, specialty diagnosis, patients' needs and the healthcare system to make informed referral decisions; managing multiple referrals, maintaining accurate records and ensuring timely follow up; ensuring patient satisfaction; and adhering to University policies and procedures as well as federal, state and local regulations.
