Job Description:
Job Summary
Essential Functions
Completes comprehensive nutrition assessments through chart review, patient interviews, anthropometric measurements, nutrition-focused physical examinations, and evaluation of clinical data.
Analyzes assessment information to identify nutrition risks, nutrition diagnoses, malnutrition indicators, learning needs, and health behaviors that may impact outcomes.
Develops, implements, documents, and monitors individualized nutrition care plans using the Academy of Nutrition and Dietetics Nutrition Care Process.
Provides medical nutrition therapy, nutrition counseling, education, and referrals as appropriate to support patient care goals.
Utilizes evidence-based counseling techniques that support health literacy, cultural sensitivity, and patient-centered care.
Monitors patient progress and evaluates the effectiveness of nutrition interventions, modifying care plans as appropriate.
Documents complete, accurate, and timely information within the electronic health record in accordance with established standards and policies.
Enters nutrition-related orders as authorized by medical staff policies and procedures.
Participates in interdisciplinary rounds and collaborates with physicians, nurses, case managers, and other healthcare professionals to develop and coordinate patient care plans.
Identifies and addresses complex discharge planning needs to support timely achievement of patient goals.
Provides nutrition expertise and consultation to clinical and foodservice teams.
Supports quality improvement initiatives, performance improvement activities, research projects, and departmental goals.
Assists in the orientation, training, and supervision of dietetic interns, registration-eligible dietitians, dietetic technicians, and new team members.
Develops, reviews, and updates educational materials, clinical resources, and departmental reference tools.
Assists with regulatory and accreditation survey preparation and compliance activities.
Responds timely to electronic communications and maintains effective communication with all stakeholders.
Performs other duties as assigned.
Knowledge, Skills, and Abilities
Ability to communicate effectively in English, both verbally and in writing.
Proficiency with Microsoft Office applications and healthcare technology systems.
Knowledge of nutrition services, medical nutrition therapy, and evidence-based dietetics practice.
Knowledge of nutrient analysis software, electronic communication tools, and spreadsheet applications.
Ability to research, evaluate, and apply current nutrition science and practice guidelines.
Ability to serve as a nutrition resource for patients, staff, and interdisciplinary healthcare teams.
Knowledge of quality improvement principles and performance improvement processes.
Ability to participate effectively in interdisciplinary meetings, patient care rounds, and care coordination activities.
Knowledge of regulatory standards, accreditation requirements, and healthcare compliance practices.
Ability to develop, implement, and evaluate educational materials and clinical resources.
Ability to identify barriers to patient care and implement solutions that improve outcomes and processes.
Proficiency in the use of electronic health record systems and clinical documentation practices.
Ability to analyze and interpret assessment data to identify nutrition-related concerns and develop appropriate interventions.
Knowledge of nutrition diagnosis development, care planning, intervention strategies, and outcome monitoring.
Ability to identify and address complex discharge planning needs.
Knowledge of National Patient Safety Goals and patient identification standards.
Ability to provide age-appropriate assessment, intervention, education, and care.
