Registered Dietitian

 

  • The Registered Dietitian will provide nutrition education to residents and assist in introducing dietary and lifestyle principles of the Dr. Bredesen program within the community.
  • Must have Skilled Nursing Experience with both Clinical and Nutrition Services. 

     

Job Description

Position Summary: We are seeking a compassionate, experienced Registered Dietitian (RD/RDN) to join our skilled nursing facility team and lead the delivery of high-quality nutrition services for our residents. The ideal candidate will provide comprehensive nutritional care through assessment, individualized care planning, monitoring, and interdisciplinary collaboration. You will help promote resident health, prevent malnutrition, and ensure compliance with regulatory requirements in accordance with California and federal standards.

Qualifications & Educational Requirements: 

  • Current Registered Dietitian Nutritionist (RD/RDN) credential.
  • Licensed in the State of California
  • Must have experience in long-term care, Skilled Nursing setting
  • Strong documentation, communication, and teamwork skills

 

Essential Duties & Responsibilities: The following duties are normal for this position. This list is not to be construed as exclusive or all-inclusive. Other duties may be required and assigned. 

  • Conduct comprehensive nutritional assessments for residents at admission and as required.
  • Develop, implement, and regularly update individualized nutrition care plans based on assessments, medical history, dietary needs, and preferences.
  • Monitor the resident's weight changes, oral intake, tolerance of therapeutic diets, and response to nutrition interventions; adjust care plans accordingly.
  • Participate in care conferences and interdisciplinary team meetings to coordinate resident nutrition goals with clinical care teams.
  • Ensure nutrition services and documentation meet CMS, Title 22, and SNF regulatory requirements, including MDS nutrition sections and menu approvals.
  • Maintain accurate and timely documentation in the resident's medical record (e.g., electronic health records or PointClickCare).
  • Approve and review menus for regular and therapeutic diets, ensuring they meet nutritional and cultural needs.
  • Provide nutrition education and counseling to residents, families, and staff on diet modifications, disease-specific nutrition, and healthy eating practices.
  • Offer ongoing training to dietary and clinical staff regarding best practices in nutrition care.
  • Other duties may be assigned by the direct supervisor.
