Registered Dietitian Nutritionist

Claxton Dietetic Solutions, LLC is seeking a motivated Registered Dietitian to join our team supporting residents in skilled nursing and rehabilitation communities throughout Northeast Ohio. This flexible part-time position offers approximately 24 hours per week, with an additional quarterly assisted living visit (approximately 4 hours per quarter).

This position is ideal for an RD who enjoys working independently while collaborating with an interdisciplinary care team to provide high-quality nutrition care and support positive resident outcomes.

Key Responsibilities 

• Complete comprehensive nutrition assessments and documentation
• Develop and update individualized nutrition care plans
• Complete MDS documentation and nutrition-related care planning requirements
• Monitor resident nutritional status, weight trends, and clinical outcomes
• Participate in interdisciplinary meetings and nutrition risk reviews
• Conduct meal observations and dining rounds
• Perform food service and sanitation reviews as needed
• Provide nutrition education and support to residents, families, and staff

Qualifications

• Registered Dietitian credential through the Commission on Dietetic Registration (CDR)
• Current state licensure/certification, as required
• Strong assessment, documentation, and communication skills
• Ability to effectively manage an independent schedule
• Experience in long-term care, rehabilitation, or geriatric nutrition preferred, but not required
