Job Description

Overview:

To provide medical nutrition therapy which will enhance the health and well being of individuals.

Responsibilities:
  • Provides medical nutrition therapy to individuals and agency staff. Develop specialized diets to aid in the management of medical conditions such as: diabetes, renal disease,    celiac disease, hyperlipidemia and other medical conditions.
  • Conducts nutritional assessments including dietary history, medical history, anthropometric measurement, biochemical assessment and pharmaceutical assessment as needed to determine nutrition requirement.
  • Completes comprehensive nutritional assessments and follow-up progress notes. Documentations are completed according to nutrition department protocol
  • Recommends specialized diets to primary care physician. Obtains signed diet orders and trains residential staff regarding implementation of the diet orders
  • Makes and returns phone calls, emails and fax’s. Tracks lab work. Follows protocol from    updating diet and sending out to appropriate team members
  • Provides for collateral intervention regarding the nutritional needs and health concerns of the individual according to the treatment plan.
  • Participates in scheduled supervisory sessions.
  • Assists in assignments as instructed by supervisor.
  • Performs other duties as requested by supervisor.

Qualifications:
  • Registered Dietitian/Nutritionist.
  • NYS Dept. of Education Certification mandatory.
  • At least one (1) year experience working with I/DD population preferred. 

This position is primarily performed in an office environment and requires regular use of standard office equipment, including computers, telephones, printers, and scanners. Essential functions involve reviewing documents, entering, and retrieving information, and communicating with employees and other stakeholders. The employee may occasionally maintain a stationary work position for periods of time, move throughout the workplace, and lift or transport materials weighing up to 20 pounds. Employees must be able to perform the essential functions of the position, with or without reasonable accommodation.
