Job Summary

The Registered Health Information Technician (RHIT) is responsible for maintaining the accuracy, integrity, confidentiality, and accessibility of patient health information. This position supports the organizations Health Information Management (HIM) functions through medical record review, chart auditing, release of information processing, document management, compliance monitoring, and data quality activities.

The RHIT works collaboratively with providers, clinical staff, operations leadership, and compliance personnel to ensure medical records meet regulatory, accreditation, payer, and organizational requirements.

Essential Functions

  • Review medical records for completeness, accuracy, and compliance with organizational standards.
  • Monitor and track incomplete medical records, provider deficiencies, and required signatures.
  • Maintain electronic and paper medical records in accordance with retention requirements.
  • Ensure timely scanning, indexing, and filing of medical record documentation.
  • Assist with chart retrieval and record management activities.
  • Process requests for medical records in compliance with HIPAA and state regulations.
  • Verify authorizations and maintain release documentation.
  • Respond to requests from patients, providers, attorneys, payers, and government agencies.
  • Track and document all release of information activities.
  • Conduct routine chart audits to ensure documentation completeness and regulatory compliance.
  • Identify documentation deficiencies and communicate findings to providers and leadership.
  • Monitor documentation standards and assist with corrective action follow-up.
  • Participate in quality improvement and compliance initiatives.
  • Maintain confidentiality of Protected Health Information (PHI).
  • Support HIPAA compliance and privacy practices.
  • Report on potential compliance concerns to management.
  • Assist with audits, investigations, and regulatory reviews as needed
  • Assist in collecting, validating, and reporting health information data.
  • Generate reports related to chart completion, record requests, and documentation deficiencies.
  • Support UDS, HRSA, payer, and compliance reporting activities.
  • Assist providers with medical record completion and documentation requirements.
  • Educate staff regarding health information management policies and procedures.
  • Provide support related to EHR documentation standards.
  • Participate in departmental meetings and training activities.
  • Assist with accreditation, licensing, and regulatory surveys.
  • Perform other duties as assigned.

Non-Essential Functions

  • As directed by the Health Information Management (HIM) Manager, performs other related and/or necessary tasks to achieve organizational and programmatic goals and objectives.

Qualifications

  • Associate's degree in health information technology, Health Information Management, or related field required.
  • Registered Health Information Technician (RHIT) credential issued by AHIMA or obtained within twelve (12) months of hire required.
  • Minimum one (1) year of experience in medical records, health information management, healthcare administration, or a related healthcare setting required.
  • Experience in an FQHC, community health center, ambulatory care clinic, or hospital setting.
  • Experience with electronic health record (EHR) systems.
  • Experience with medical record auditing and release of information processes.

Knowledge, Skills and Abilities

§ Knowledge of HIPAA, privacy regulations, and health information management principles.

§ Understanding of medical terminology and healthcare documentation standards.

§ Knowledge of medical records retention and release of information requirements.

§ Strong attention to detail and organizational skills.

§ Ability to analyze records for completeness and accuracy.

§ Excellent communication and customer service skills.

§ Proficiency with EHR systems, Microsoft Office, and healthcare technology platforms.

§ Ability to maintain strict confidentiality.

Travel

  • Will be travelling to other clinic's sites to work or attend meetings

Physical Requirements and Environmental Conditions

  • Pushing and pulling objects up to 25 lbs.
  • Prolonged periods of sitting, standing, and computer use.
  • Frequent wrist, hand and finger dexterity to perform fine motor function without tremor.
  • Full range of body motion including twisting body, pushing, and pulling.
  • Position requires light to moderate work with 25 lb. maximum weight to lift and carry.
  • Position requires reaching, bending, stooping, and handling objects with hands and/or fingers, talking and/or hearing, and seeing.

Work Schedule

  • You will generally be scheduled to work each day Monday through Friday, with starting times each day varying between 8:00 a.m. and 9:00 a.m. and ending time between 5:00 p.m. and 6:00 p.m., as required to meet the operational needs of the company. There will be occasional Saturdays as needed.

You will need to travel to other clinic locations as needed for cross coverage support.
