SUMMARY: As a RN Level 2 you provide direct patient care and case management within a patient and family centeredcare environment. The registered nurse case manager makes the initial evaluation visit, regularly reevaluates the patient's nursing needs, initiates the plan of care and necessary revisions, furnishes those services requiring substantial and specialized nursing skill, initiates appropriate preventive and rehabilitative nursing procedures, prepares clinical and progress notes, coordinates services, informs the physician and other personnel of changes in the patient's condition and needs, counsels the patient and family in meeting nursing and related needs, participates in in-service programs, and supervises and teaches other nursing personnel.

 

Schedule: Monday–Friday, 8:00 AM–4:30 PM. Participation in the on-call rotation is required approximately 2-3 days per month, 3 Holidays/ a year.

Sign On Bonus: up to $15,000

Location: Serving Wayne, Seneca, Ontario & Yates Counties

 

Education & Experience - Applicants must possess the following qualifications or an equivalent combination of education and experience:

  • Education: New York State Licensed RN
  • Experience: One year of experience as a clinical nurse in a hospital and nine months experience in home care preferred.
  • Licensure: New York State Licensed RN

 

Key Responsibilities

  • Provide comprehensive patient assessments, case management, and skilled nursing care in the home.
  • Develop, implement, and revise individualized care plans in collaboration with patients, families, physicians, and the interdisciplinary team.
  • Complete OASIS assessments, clinical documentation, and regulatory requirements accurately and on time.
  • Coordinate care, communicate changes in patient condition, and educate patients and caregivers on disease management and self-care.
  • Supervise LPNs and ensure the delivery of safe, high-quality patient care.
  • Support quality improvement initiatives, maintain productivity standards, and participate in ongoing professional development.

 

Salary Range: $73,652 to $103,854
