The Contract Nurse is a clinical care team member operating within a Value-Based Care model focused on improving patient health outcomes and long-term wellness. The Nurse supports patients by utilizing clinical standards, guidelines, and care pathways to prevent complications and reduce avoidable hospitalizations.
The Nurse will perform clinical assessments, support chronic condition management, collaborate with interdisciplinary care teams, and provide patient-centered care through effective communication and clinical coordination.
Core Responsibilities- Support patient assessment and prioritization for telephonic triage to ensure patients receive the appropriate level of care.
- Perform protocol-driven chronic condition management.
- Provide comprehensive patient and caregiver education regarding chronic conditions and preventive care over the phone.
- Conduct medication reviews during nurse-led visits to promote patient safety and medication adherence.
- Review and appropriately act upon high-priority abnormal laboratory results.
- Utilize standardized protocols for medication management, prescription renewals, and prior authorizations.
- Ensure clinical communications and assigned tasks are completed accurately and timely.
- Complete and close patient visit encounters in a timely manner.
- Perform other related duties as assigned.
Clinical Responsibilities- Support the coordination, integration, communication, and implementation of nursing practices and standard operating procedures.
- Identify barriers to optimal care, including physical, cognitive, language, literacy, socioeconomic, cultural, and behavioral factors.
- Create a welcoming and engaging environment that meets the needs of patients, families, communities, and care teams.
- Deliver an exceptional patient experience through responsive, respectful, and patient-centered care.
- Participate as a key clinical resource during daily care team meetings.
- Serve as a support and resource for nursing staff, clinical team members, and providers.
- Ensure compliance with patient safety initiatives and reporting requirements.
- Provide emergency care in accordance with organizational procedures.
Professional Responsibilities- Participate in self-evaluation and professional development activities.
- Maintain current nursing knowledge and clinical competency.
- Stay informed about current patient-centered and evidence-based practices.
- Apply appropriate research and clinical data to patient care delivery.
- Identify and report professional misconduct appropriately.
Required Qualifications- Active, unrestricted Registered Nurse (RN) license in the state of residence.
- Candidate must reside in Illinois.
- Current BLS or CPR certification.
- Fluent written and spoken English.
- Authorized to work in the United States.
- Experience with Electronic Medical Records (EMR).
- Ability to maintain patient confidentiality.
- Strong clinical assessment and communication skills.
- Ability to assess patients effectively without face-to-face interaction.
Strongly Preferred Qualifications- Experience collaborating with interdisciplinary care teams.
- Strong computer skills with the ability to interpret and analyze information from various systems.
- Excellent problem-solving, prioritization, and organizational skills.
- Bilingual proficiency, particularly Spanish.
Preferred Qualifications- Experience with Epic electronic health record systems.
- Previous clinical nursing experience.
- Ability to work independently.
- Experience working within Value-Based Care or population health models.
- Experience using Google Sheets for data management and reporting.
