    Job Title: Registered Nurse – Acute Rehabilitation
    Location: Peoria, Illinois
    Schedule: Full-Time | 12-Hour Night Shifts
    Pay Range: $32.94–$49.36 per hour
    Night Differential: +$4.00 per hour
    Weekend Differential: +$3.25 per hour
    Sign-On Bonus: $20,000 with a one-year commitment

    Job Overview

    A leading healthcare organization is seeking an experienced Registered Nurse to join its Acute Rehabilitation team in Peoria, Illinois.

    The Acute Rehab RN will provide comprehensive nursing care to patients recovering from serious illness, injury, surgery, neurological conditions, and other complex medical events. Working closely with physicians, therapists, nursing staff, patients, and families, the RN will help patients progress toward greater independence and a safe transition to the next level of care.

    This position is ideal for a nurse who enjoys interdisciplinary teamwork, patient education, rehabilitation-focused care, and building longer-term therapeutic relationships with patients and their families.

    Responsibilities

    • Provide direct nursing care to patients receiving acute rehabilitation services
    • Complete comprehensive patient assessments and ongoing reassessments
    • Develop, implement, and evaluate individualized nursing plans of care
    • Collaborate with physicians, therapists, case management, and other members of the interdisciplinary rehabilitation team
    • Coordinate nursing care with each patient's overall rehabilitation goals
    • Monitor patients for changes in clinical condition and communicate concerns promptly
    • Administer medications, treatments, and other nursing interventions according to provider orders and established protocols
    • Support patients with mobility, functional independence, self-care, and recovery goals
    • Provide emotional, psychological, and spiritual support to patients and caregivers
    • Educate patients and families regarding medications, treatments, disease management, rehabilitation goals, and discharge planning
    • Advocate for patients and caregivers throughout the healthcare continuum
    • Maintain accurate, timely, and complete documentation in the electronic medical record
    • Protect patient privacy and maintain confidentiality of medical information
    • Delegate appropriate nursing responsibilities based on staff competency and scope of practice
    • Provide leadership and guidance to nursing team members
    • Assist with orientation and mentoring of new employees
    • Participate in quality-improvement and process-improvement initiatives
    • Promote evidence-based nursing practices and positive clinical outcomes
    • Maintain a safe, clean, and supportive patient-care environment
    • Utilize resources responsibly while maintaining high standards of care
    • Maintain professional knowledge through continuing education and awareness of current clinical, regulatory, and accreditation standards
    • Perform other nursing responsibilities as assigned

    Qualifications

    Required

    • Graduate of an accredited Registered Nursing program
    • Current Registered Nurse license appropriate for practice in Illinois
    • Current American Heart Association Healthcare Provider Basic Life Support certification prior to start
    • At least one year of nursing experience in a rehabilitation or related patient-care environment
    • Strong clinical assessment and nursing decision-making skills
    • Ability to prioritize multiple patient-care responsibilities
    • Strong written and verbal communication skills
    • Ability to work collaboratively within an interdisciplinary rehabilitation team
    • Ability to provide compassionate care to patients with complex physical and emotional needs
    • Ability to work 12-hour night shifts

    Preferred

    • Bachelor of Science in Nursing
    • Previous acute rehabilitation nursing experience
    • Experience caring for medically complex rehabilitation patients
    • Proficiency with electronic medical records
    • Basic proficiency with Microsoft applications
    • Experience providing patient and family education
    • Previous charge, preceptor, or mentoring experience

    Physical & Clinical Requirements

    • Ability to move frequently throughout patient-care areas
    • Ability to stand or remain stationary for extended periods
    • Ability to frequently lift, move, or assist with patients and objects weighing up to approximately 35 pounds
    • Ability to operate medical and office equipment throughout the shift
    • Ability to respond appropriately to rapidly changing clinical situations
    • Ability to function effectively in emotionally stressful environments
    • Potential exposure to bloodborne pathogens, communicable diseases, and healthcare-related chemicals
