Responsibilities:

The primary purpose of this position is to provide clinical assessments, develop resident care plans, and supervise Home Health Aide (HHA) staff within the Assisted Living Program. The professional nurse builds from the resources of the community to plan, and direct services to meet the needs of the residents and families within their home.

Essential Job Functions

  • Perform initial, ongoing, and mandatory 6-month Uniformed Assessment System (UAS) assessments of residents.
  • Develop, implement, and update Plans of Care (POC) based on assessment.
  • Directly supervise and instruct Home Health Aides (HHA).
  • Plan, direct, and evaluate the training for the Home Health Aide students.
  • Assist the HHA students in the ability to transfer classroom knowledge to the clinical setting.
  • Maintain compliance with LHCSA and NYS Department of Health regulations.
  • Complete accurate clinical notes and assessments.
  • Coordinate with physicians, families, and staff regarding resident condition changes and medication reconciliation.
  • Develop a care plan that keeps the resident as independent as possible.
  • Provide education to residents, families, and home health aides.
  • Conduct bi-monthly Home Health Aide skill checks.
  • Supervise and counsel nursing staff.
  • Assist with the monthly medication self-administer assessments.
  • Assist with monthly case management documentation.
  • Assist with facilitating monthly resident care conference meetings.
  • Assist with admissions and discharges to/from the Enriched Housing and Assisted Living Programs.
  • Be an active member of the Quality Improvement Committee, attend quarterly meetings, and perform quality assurance audits.
  • Rotate on-call schedule with other clinical staff.
  • Cover for the Adult Day Healthcare RN in their absence.
  • Any other tasks or duties as assigned.
Qualifications:

Education:

  • 2 year/ Associate Degree - Registered Nurse; required to obtain Bachelor's degree or higher within 10 years of licensure

Experience:

  • Minimum 5 years in human services or related field, preferred

Licensure/Certifications:

  • NYS License as a Registered Nurse, required
  • BLS Certification within 6 months from date of hire, required

Physical Requirements:

  • Light Work - Exerting up to 20 lbs of force frequently, and/or a negligible amount of force constantly to move objects. If the use of arm and/or leg control requires exertion of forces greater than that of sedentary work and if the worker sits most of the time, the job is considered light work.
  • Occasional balancing, crouching, grasping, kneeling, lifting, pulling, pushing, reaching, repetitive motion, standing, stooping, walking, and fit testing
