Pride Health is hiring for an RN Care Coordinator for a client in Houston, TX! (Contract)

Schedule: Mon-Fri 8-5pm

Contract: 3 month contract

Pay Rate: $40-45/hour

 

 

Specific Duties:

  • Setting: Family Medicine
  • Coordinate patient care plans across providers, departments, and services.
  • Perform clinical triage via phone, EMR messaging, or in person.
  • Review labs, imaging, and clinical results and escalate appropriately.
  • Prepare patients for procedures and ensure pre op requirements are met.
  • Educate patients on treatment plans, medications, and follow up care.
  • Manage referrals and authorizations (especially in outpatient settings).
  • Document all clinical interactions in the EMR accurately and timely.
  • Serve as primary point of contact between patient, family, and care team.
  • Monitor patient progress and intervene when care gaps appear.
  • Support quality improvement initiatives within the department.

 

Must have qualifications/experience:

  • Bachelor's Degree in Nursing preferred
  • RN License
  • BLS Certification
  • Experience in Family Medicine
  • Strong patient education and care coordination skills
  • EMR proficiency (Epic)
  • Experience working in multidisciplinary teams
  • Ability to manage high patient volumes and complex cases
  • Care coordination or case management experience (1+ year preferred)
