Job Description

UVA Health is seeking an experienced Registered Nurse Care Coordinator to help advance one of UVA's newest and most rapidly growing specialty programs: the Ehlers-Danlos Center. This is a rare opportunity to help shape a developing multidisciplinary clinic designed for patients with Ehlers-Danlos Syndrome, related hypermobility disorders, vascular EDS, and complex multisystem chronic conditions.

This RNCC role is central to how patients'' access, navigate, and experience care. As the clinic continues to expand, the RN Care Coordinator will serve as a key connector across providers, specialties, patients, and families, helping ensure that each patient's journey from referral through follow-up is organized, responsive, and clinically informed.

The Role

The Registered Nurse Care Coordinator serves as the central clinical coordinator for a growing specialty clinic that supports adolescents and adults with complex, often lifelong conditions. This role requires strong assessment skills, exceptional organization, and the ability to coordinate across multiple disciplines in a fast-moving ambulatory environment.

The schedule is Monday through Friday, 8:00 AM to 4:30 PM, with no weekends, no holidays, and no call responsibilities. This position is currently on-site five days per week.

On an average day, you may be reviewing incoming referrals, gathering outside records, triaging patient calls, preparing the care team for upcoming visits, and helping patients and families understand the next steps in their care. You will work closely with attending physicians, nurse practitioners, an LPN, access team members, a registered dietitian, behavioral health specialist, genetic counselor, and specialty partners across UVA Health.

Key responsibilities include:

  • Managing referral intake and care coordination from initial referral through ongoing follow-up.

  • Reviewing incoming referrals and ensuring required documentation and outside medical records are available for provider review.

  • Triaging patient calls and appointment requests using strong nursing judgment and patient assessment skills.

  • Coordinating multidisciplinary care plans for patients who may need involvement from cardiology, physical therapy, occupational therapy, behavioral health, nutrition, genetics, ophthalmology, gastroenterology, and other specialty services.

  • Supporting clinic preparation by helping providers and ancillary teams stay informed, organized, and ready for complex visits.

  • Responding to patient and family questions with clarity, compassion, and strong family-centered communication.

  • Facilitating scheduling-related coordination and supporting provider workflows in a high-demand specialty clinic.

  • Helping build future workflows and processes as the Ehlers-Danlos Center continues to grow.

This is an excellent fit for an RN who enjoys building new programs, thrives in a fast-growing clinical environment, and is comfortable partnering across multiple specialties. Pediatric nursing experience is strongly preferred, especially experience working with pediatric patients and families. Ambulatory nursing experience in genetics, rheumatology, neurology or complex care would be valuable to this role. Additionally, care coordination and Epic familiarity is preferred.

Why UVA Health

At UVA Health, you will join an integrated academic health system with a world-class academic medical center, a Level I trauma center, a Level IV NICU, Virginia's first NCI-designated Comprehensive Cancer Center, and UVA Health Children's, recognized as the #1 pediatric hospital in Virginia.

This role is especially compelling because it is tied to a new and expanding specialty center with significant patient needs and long-term growth potential. The Ehlers-Danlos Center officially opened in December and continues to expand rapidly. The clinic is building a comprehensive, integrated model of care designed to bring specialists together in a coordinated way for patients with EDS and related hypermobility disorders.

Why this opportunity stands out:

  • Help build one of UVA's newest specialty clinics from the ground floor.

  • Play a key role in a multidisciplinary care model that connects patients with multiple specialty services across UVA.

  • Work with nationally recognized expertise in Ehlers-Danlos Syndrome, with the center led by Dr. Dacre Knight, who joined UVA from Mayo Clinic Florida.

  • Collaborate with a broad specialty team including physicians, nurse practitioners, behavioral health, nutrition, genetics, rehabilitation, cardiology, and other clinical partners.

  • Contribute to innovation in patient care, including new nurse-led group visit models designed to improve patient access, education, and engagement.

  • Make meaningful patient impact by helping patients with complex, multisystem conditions access specialized resources and improve quality of life.

  • Join a growing program that is expanding providers, services, staffing, and program offerings to meet exceptionally high patient demand.

  • Benefit from a stable outpatient schedule with no call, no weekends, and no holidays.

Relocation assistance may be available for qualified candidates.

This is more than a care coordination position. It is an opportunity to help define how a new specialty center delivers care, improves access, and supports patients who often need thoughtful, coordinated, whole-person clinical navigation.
