Responsible for education coordination within the Care Management Department and responsible for assisting with grant development and grant implementation within Care Management.
MINIMUM SPECIFICATIONS
Education
Must have a Master's degree in Social Work from an accredited school of social work or a degree in Nursing from an accredited school of nursing.
Experience
Must have two (2) years of hospital based experience to include one (1) year of experience of grant development and staff education.
Certification/Registration/Licensure
Must be currently licensed as a Licensed Master Social Worker by the Texas State board of Social Work Examiners. 'OR'
Must be currently licensed as an RN by the Texas Board of Nurse Examiners or have a valid temporary permit with the Texas Board of Nursing or valid Compact RN license.
Skills or Special Abilities
Must have excellent oral, written and organizational skills.
Must have excellent interpersonal skills and be able to communicate and collaborate effectively with a diverse group of patients, families and healthcare staff.
Must have grant-development skills.
Must be detail oriented, self-directed, and capable of priority-setting and problem solving.
Must be able to demonstrate a working knowledge of PC operations and the ability to use work processing software in a Windows environment.
Must be able to demonstrate patient centered/patient valued behaviors.
Responsibilities
Ensures familiarity with the standards of professional conduct duties in accordance with the Rules of the Texas State Board of Social Work Examiners and the National Association of Social Workers Code of Ethics, as well as, those of the Board of Nurse Examiners for the state of Texas.
Provides ongoing educational needs assessments using a variety of sources that reflect the organizational environment and healthcare trends and individual learning characteristics.
Conducts orientation for department employees to ensure staff maintains knowledge of trends, developments and advancements in practice and the policies and procedures of Parkland.
Coordinates or facilitates educational offerings pertinent to Care Management topics and clinical issues.
Maintains relationships with multiple department leadership in collaborating on projects related to regulatory accreditation and/or findings.
Trains staff and works collaboratively with management to implement process improvements in support of the overall goals of the department and Parkland.
Provides leadership and assistance to departmental managers with the identification, development, implementation and evaluation of grant proposals to ensure compliance with grant requirements and information on the budgetary impact on Care Management. Educates program directors, managers and other Parkland personnel as necessary.
Assists with problem solving around specific grant issues, balancing meeting grant objectives and compliance requirements with the goals of Parkland and Care Management. Manages in-house donations to Care Management by the Foundation or the Auxiliary as available.
Cultivates relationships with outside sponsors and program directors to encourage more grant opportunities.
Serves on various interdisciplinary committees as assigned to provide grant expertise and representation.
Provides project support to Care Management department as assigned
Job Accountabilities
Identifies ways to improve work processes and improve customer satisfaction. Makes recommendations to supervisor, implements, and monitors results as appropriate in support of the overall goals of the department and Parkland.
Stays abreast of the latest developments, advancements, and trends in the field by attending seminars/workshops, reading professional journals, actively participating in professional organizations, and/or maintaining certification or licensure. Integrates knowledge gained into current work practices.
Maintains knowledge of applicable rules, regulations, policies, laws and guidelines that impact the area. Develops effective internal controls designed to promote adherence with applicable laws, accreditation agency requirements, and federal, state, and private health plans. Seeks advice and guidance as needed to ensure proper understanding.
