Job Description
Elitecare Medical Staffing is seeking a dedicated and experienced Registered Nurse - Case Manager to join our dynamic team in Concord, CA. The ideal candidate will have recent experience in acute care hospital case management and a passion for delivering exceptional patient care.
Key Responsibilities:
- Provide comprehensive case management services to patients in an acute care hospital setting.
- Float between Concord and Walnut Creek as needed, adhering to the local travel policy (50-mile radius).
- Collaborate with healthcare providers, patients, and their families to develop and implement effective care plans.
- Utilize the Epic Electronic Medical Record (EMR) system for documentation and communication.
- Ensure compliance with all regulatory requirements and organizational policies.
- Participate in interdisciplinary team meetings to discuss patient progress and adjust care plans as necessary.
Qualifications:
- Licensure: Current California Registered Nurse (RN) license required at the time of submission.
- Experience: Minimum of 2 years of recent acute care hospital case management experience is mandatory. Previous travel nursing experience is preferred.
- Skills: Proficient in using electronic medical records, preferably Epic. Strong communication and interpersonal skills are essential.
- Ability to work collaboratively in a fast-paced environment while managing multiple priorities.
