Position Title: Registered Nurse – Case Manager
Location: Eureka, California
Pay Rate (Local): Up to $55.00/hour
Pay Rate (Travel): Up to $70.00/hour
Weekly Hours: 40 hours
Schedule: Night Shift (5x8s) | 07:00 PM – 07:00 AM | Every other weekend
Assignment Duration: 13 weeks (with potential for extension)

Position Overview

"Navitas Healthcare, LLC" is seeking an experienced Registered Nurse – Case Manager to support the Case Management team within an acute care hospital setting. This role is focused on care coordination, discharge planning, utilization management, and ensuring appropriate patient flow across the continuum of care.

Key Responsibilities

• Coordinate patient care and discharge planning across the continuum of care
• Conduct comprehensive needs assessments and develop individualized care plans
• Review admission criteria, insurance benefits, and prior authorization requirements
• Manage utilization review activities including appeals and denial management
• Collaborate closely with interdisciplinary teams including physicians, nursing, and social services
• Ensure compliance with CMS regulations, HIPAA standards, and hospital policies
• Facilitate safe patient transitions and coordinate durable medical equipment (DME) needs
• Maintain accurate documentation of case management activities in Epic

Required Qualifications

• Minimum of 2 years of acute care hospital Case Management experience
• Strong background in care coordination and discharge planning
• Recent clinical experience within the last 12 months

Education & Licensure

• Associate Degree in Nursing (ADN) or higher
• Active Registered Nurse (RN) license in the State of California
• BLS certification preferred
