What You’ll Do
- Client Assessments & Care Plans – Evaluate needs and design personalized care plans
- Coordinate Services – Partner with nurses, caregivers, and providers for seamless support
- Advocate for Clients – Be a trusted voice for patients and families
- Maintain Compliance – Keep records accurate and services aligned with state and federal guidelines
- Crisis Support – Step in quickly when urgent issues arise
- Monitor & Adjust – Track progress and adapt care plans to improve outcomes
What We’re Looking For
- Excellent communication & relationship-building skills
- Strong organization and time-management abilities
- Knowledge of Medicaid, Medicare, and Indiana home care guidelines
- Background in case management, social work, or healthcare
- Tech-savvy with EHR or case management systems
- Bilingual (English/Burmese) is a plus—immediate need!
