- Shift: 8:00 AM 4:30 PM
Required Qualifications Licensure & Certification
- Active California RN License
- Current BLS Certification
Experience
- Inpatient Acute Care Case Management experience required
- Complex discharge planning experience required
Key Responsibilities
- Coordinate patient care across the continuum from admission through discharge.
- Develop and manage individualized discharge plans for complex patient populations.
- Collaborate with physicians, nurses, therapists, social workers, and community agencies.
- Monitor appropriateness and timeliness of care delivery.
- Evaluate resource utilization and facilitate efficient patient progression.
- Coordinate transfers to tertiary care facilities and ensure continuity of care.
- Review treatment plans, diagnostic testing, and discharge needs.
- Participate in interdisciplinary rounds and care conferences.
- Identify barriers to discharge and implement solutions.
- Ensure compliance with regulatory and accreditation standards.
- Maintain accurate case management documentation.
- Support patients and families throughout hospitalization and discharge planning.
