Job Title: Registered Nurse Case Manager

Location: Grand Blanc, MI

Assignment Type: 13-week contract (with the option to extend)

Schedule: 4 days a week / 10 hour shifts 7:30am – 6pm

Position Overview:

We are currently seeking a highly Experienced Acute Care Inpatient Case Manager with strong discharge planning expertise to join our team. This role requires professionals who can step in with a minimal training, adapt quickly, and manage patients efficiently in a fast-paced Acute Care Environment.

Key Responsibilities:

·         Facilitation of precertification and payor authorization processes.

·         Facilitation of the collaborative management of patient care across the continuum, intervening as necessary to remove barriers to timely and efficient care delivery and reimbursement.

·         Application of process improvement methodologies in evaluating outcomes of care.

·         Support and coaching of clinical documentation efforts and serving as a clinical resource for coders ensuring that documentation accurately reflects severity of illness and intensity of service.

Requirements:

·         Minimum 5 years acute Inpatient Case Management Experience

·         Experience in acute discharge planning

·         BSN Required

·         BLS certification through AHA

·         Previous experience using CarePort system.
